"My CD" Electronic Advance Directive Program

“My CD”, short for My Care Decisions, is a revolutionary tool that enables you to take control of your life. This advance directive program is a pocket-sized compact disk that holds all of your medical care preferences, such as your living will, and other important legal documents. The disk fits easily into a wallet or purse for portability and piece of mind so that you may carry it wherever you go. My CD is readable on any computer and can be viewed at hospitals worldwide. In the event of an emergency, you and your loved ones can find comfort in knowing that your wishes will be carried out.

Advance directive laws and forms vary by state. “My CD” is a program of the Center for Health Futures at Florida Hospital, a 501(c)3 non-profit organization. A minimal charge of $5 covers the cost of materials, postage and administration.
